The Arc of the Piedmont is looking for Sponsored Residential providers, as well as Back-Up Sponsored Residential providers. This is a very exciting opportunity for the entire agency. The Arc of the Piedmont connects adults who have intellectual disabilities with families who take on the role of Family Sponsor. The Family Sponsor offers their home, time and support to an individual in daily living and skills that will help him or her achieve personal goals, independence and wider social supports. Sponsored Residential Providers are not direct employees of The Arc of the Piedmont; rather, they are contract employees.

In order to be considered for licensing to provide services, candidates must be found eligible based on criminal history background checks and Child Protective Services registry checks. Candidates must also meet requirements for minimum knowledge, skills and abilities according to their job description. For sponsored residential positions, the requirements are a total of one year direct experience working with persons with intellectual disabilities or developmental disabilities or a bachelor’s degree in psychology, social work, special education or related field. Completion of CPR, First Aid, College of Direct Supports within the first six months, medication administration. Training and certification are available through The Arc of the Piedmont.

Back-up Residential Service providers serve as the second line of support for primary Sponsored Residential providers. In the event that the primary provider cannot fully attend to the needs of the person being supported (owing to other occasional commitments, illness, work, need for time off) the Back-Up provider then provides necessary services and support. We happily pair back-up providers to primary providers.

Becoming a sponsored provider is a two-fold process. Both the individual who wants to become a sponsored provider and their home must meet standards. After an assessment of the candidate’s capabilities, experience and training, and financial capacity to meet expenses, and after it is determined that the home is suitable and meets requirements, the sponsored provider is licensed through the Arc of the Piedmont to provide care in their home.  Placement is contingent upon the desire of the individual who is seeking placement.

The Direct Support Professional (DSP) for The Arc of the Piedmont is responsible for the implementation of person-centered plans through teaching and assisting the people we support in a community based residential setting, or in one of our Adult Activity Program settings. DSPs maintain required documentation and reporting, assist with personal care as needed, foster a warm and caring home environment and initiate in-home and community based recreation activities. A high school diploma or equivalent is required and, as always, we seek creative innovative individuals with an eye towards providing the greatest quality of life possible for the people we support. JOB DUTIES:

  1. Implements the Individual Service/Program Plans by effectively teaching and supporting individuals as outlined in the plan.  Applies positive, non-aversive approaches (i.e. Safety-Care, Positive Behavior Supports) to providing support.
  2. Observes individual participation and progress in meeting Individual Service or Program Plan’s goals and objectives.  Assists the Day Support Manager in the development and/or revision of Plans to better meet each individual’s needs.
  3. Assures a caring, stimulating program environment.  Initiates projects that create a supportive program environment, involving individuals in this process to the furthest extent possible for each individual.  Provides recommendations to the Day Support Manager for program changes.
  4. Maintains daily, weekly and other regularly scheduled documentation and reports regarding individual activities, programs and their progress.  Maintains required documentation within specified timeframes. Uses professional, person-centered language in the completion of all documentation.
  5. Assists individuals with medication administration to assure accuracy.  Monitors clients to assess medication effectiveness, side effects and reports observations to Day Support Manager for follow up.  Notes and reports errors in medication administration.
  6. Plans and initiates a variety of on-site and community-based recreation activities, which incorporate individual preferences and skills as well as concepts of self- direction, interpersonal competence, and community engagement.
  7. Assures the safety of individuals and makes reasonable efforts to protect individuals from harm, abuse, neglect, and exploitation.  Immediately reports any concerns or observations of potential harm, abuse, neglect, or exploitation.
  8. Utilizes necessary equipment to assure the safe transfer of clients, as well as protects oneself from personal injury (i.e. training and use of the Hoyer Lifts.)
  9. Assures the program facility is safe and secure. Immediately reports any safety concerns identified and takes action to address any concerns. Utilizes and contacts emergency services including local law enforcement as needed.
  10. Provides safe transportation to clients when necessary for daily commuting, employment, healthcare needs, and functions of community life.
  11. Attends staff meetings as scheduled Attends other trainings and meetings as required to maintain knowledge, skills and abilities.
  12. Provides residential day support to homebound Adult Activity Center participants when the Adult Activity Center is closed or delayed due to inclement weather.
  13. May involve, as required, personal care which may include some or all of the following: toileting, changing briefs, bathing and feeding.

Skills:  safe operation of passenger vehicles as evidenced by a valid Virginia driving license and acceptable Department of Motor Vehicle driving record, CPR/First Aid procedures, crisis prevention, positive behavioral supports, intervention and emergency responses, documentation

Abilities: to lift and transfer at least 50 pounds and, for transfers of the people we support, utilize appropriate equipment (i.e. Hoyer Lift) to prevent personal injury or injury to the client, work independently; follow directions; communicate verbally and in writing, interact effectively and courteously with a variety of people, teach effectively, provide personal assistance

Quality Assurance and Quality Improvement (QA/QI) Specialist

GENERAL STATEMENT OF RESPONSIBILITIES:

The Quality Assurance and Quality Improvement (QA/QI) Specialist will ensure quality monitoring and improvement, program evaluation tasks, and agency planning processes.

This is a professional level exempt position reporting to the Deputy Executive Director of Community Support Services. The QA/QI Specialist performs in accordance with professional ethics and established policies.  They are expected to function with substantial initiative as well as utilize independent judgement.

 

Job Duties:

  1. Works to ensure agency compliance (to include licensure reviews) with the Department of Behavioral Health and Developmental Services (DBHDS), the Department of Medical Assistance Services (DMAS), the Office of Human Rights, Library of Virginia, and agency policies and regulations.
  2. Collaborates with Program Managers/Directors to write and/or advise in the development and revision of Individualized Support Plans (ISPs) to ensure the provision of high quality individualized services consistent with the needs and preferences of the individuals receiving those services.
  3. Ensures that all individual records are current and complete as well as in compliance with state regulations and agency policies. Facilitates on-going assessments of documentation requirements throughout the agency to ensure that requirements are met utilizing the most efficient process possible.
  4. Reviews and approves all ISPs.
  5. Completes necessary reports using DBHDS’ Computerized Human Rights Information System (CHRIS) as well as attending to service authorizations through the Waiver Management System (WaMS) on a needed basis.
  6. Reviews service dates for the reimbursement billing process.
  7. Provides administrative guidance and sets up controls for the regular review of individual records and the systematic collection and posting of data. Prepares periodic reports, including Individual Support Plans according to organizational procedures and regulatory compliance statutes. Ensures maintenance of individual records.
  8. Develops and implements initiatives as recommended by the Quality Assurance Committee while acting as a key member of the QA committee.
  9. Conducts Human Rights agency investigations as needed.
  10. Maintains close liaisons with agency administration and service delivery site as well as a professional liaison with designated oversight agencies.
  11. Serves as records retention officer to verify the retention and deposition of agency records in accordance to the Library of Virginia regulations.
  12. Provides scheduled and unscheduled compliance reviews of all programs.
  13. Works cooperatively with Program Managers and Directors on areas of agency compliance and utilization review of quality assurance standards including serving as an agency resource to staff in the development and implementation of corrective activities.
  14. Provides status reports to the Deputy Executive Director of Community Support Services and record evaluation reports for agency management.
  15. Delivers documentation training of new hired staff with a particular focus on quality of service and compliance terminology, tasks, and outcomes as they pertain to the greater context of the agency’s mission (i.e. the importance of documentation, the standards of documentation, how that interrelates with reimbursements for services, auditing, etc.)
  16. Performs other job related duties as assigned by the Deputy Executive Director of Community Support Services.

 

REQUIRED KNOWLEDGE, SKILLS AND ABILITIES:

Knowledge of:  developmental disability support systems, person-centered planning, community integration, service coordination, and governmental regulations as they pertain to services for individuals with developmental disabilities; principles of quality assurance, risk management, and performance improvement techniques.

In addition: knowledge of incident reporting, assessments, and service authorization processes, as well as knowledge of DBHDS licensing regulation, Human Rights regulations and HCBS regulations.

Skills:  strong organizational skills, effective monitoring of CHRIS and WaMS

Ability:  to interact well with staff as well as outside agencies and resources, to develop and implement guidelines, protocols, and best practices.

 

SUGGESTED TRAINING AND EXPERIENCE:

Bachelor’s Degree in a human services related field with three years of experience or a combination of education, training, and experience providing the required knowledge, skills, and abilities.

 

SCHEDULE: 40 hours per week
